Notice of Privacy Practices

This notice describes how medical and mental health information about you may be used and disclosed, and how you can get access to this information. Please read it carefully.

This website does not collect protected health information. Any clinical information is gathered only within secure, HIPAA-compliant systems after you become a client.

How your information may be used and disclosed

Treatment. Protected health information (PHI) may be used to provide, coordinate, or manage your care. With your written authorization, information may be shared with other providers involved in your treatment, such as a physician, dietitian, or psychiatric prescriber.

Payment. This is a self-pay practice. PHI may be used to prepare invoices, receipts, or out-of-network superbills that you request and submit to your insurer.

Health care operations. PHI may be used for quality review, clinical supervision, consultation, record keeping, and administration of the practice. Clinical supervision is a required and ongoing part of practice as a New York LMSW, and supervisors are bound by the same confidentiality obligations.

Your rights

  • The right to inspect and receive a copy of your record, with limited exceptions.
  • The right to request an amendment to information you believe is inaccurate or incomplete.
  • The right to an accounting of certain disclosures of your information.
  • The right to request restrictions on how your information is used or disclosed. Not every request can be granted, but each one is considered.
  • The right to request confidential communications, including contact at a specific phone number, email address, or mailing address.
  • The right to a paper copy of this notice on request, even if you agreed to receive it electronically.
  • The right to revoke a prior authorization in writing, except where action has already been taken in reliance on it.

Legal duties of this practice

This practice is required by law to maintain the privacy of your protected health information, to provide you with this notice of legal duties and privacy practices, and to abide by the terms of the notice currently in effect. If the notice is revised, the revised version will apply to all information held at that time, and the current version will be made available on request and posted on this site.

Telehealth

Telehealth sessions across New York are delivered over secure, encrypted, HIPAA-compliant platforms with a signed business associate agreement in place. Email and text are not secure channels and are used only for scheduling and administrative logistics, never for clinical content.

Confidentiality and its limits in New York

New York law protects the confidentiality of communications between a client and a licensed social worker. Information is disclosed without your authorization only where the law requires or permits it, including:

  • When there is an imminent risk of serious harm to yourself or to another identifiable person.
  • When there is reasonable cause to suspect abuse or neglect of a child or of a vulnerable adult.
  • When disclosure is compelled by a court order or other lawful legal process.

Where a disclosure is required, only the minimum information necessary is shared, and you will be told about it whenever it is safe and possible to do so.

Complaints

If you believe your privacy rights have been violated, you may file a complaint directly with the practice through the contact form. You may also file a complaint with the U.S. Department of Health and Human Services, Office for Civil Rights, at 200 Independence Avenue SW, Washington, DC 20201, or at hhs.gov/ocr/privacy/hipaa/complaints. You will never be penalized or retaliated against for filing a complaint.

Effective date

This notice is effective as of August 2026 and remains in effect until it is replaced.
